ABSTRACT

Abstract Livestock is a fundamental part of the agriculture industry in Pakistan and contributes more than 11.53% to GDP. Among livestock species, the buffaloes are regarded as the black gold of Pakistan. Being the highest milk producers globally, Nili-Ravi buffaloes are the most famous ones. Buffaloes are affected by many endemic diseases, and "Hemorrhagic septicemia" (HS) is one of them. This study was designed to ascertain the effects of experimental exposure ofP. multocida B:2 (oral) and its immunogens, i.e., LPS (oral and intravenous) and OMP (oral and subcutaneous) on reproductive hormonal profiles in Nili-Ravi buffaloes. Repeated serum samples were collected from the jugular vein of experimental animals for 21 days (0, 02, 04, 08, 12, 16, 20, 24, 36, 48, 72, 120, 168, 216, 264, 360, 456 and 504 hours). Hormonal assays to determine the serum concentrations of Gonadotropin-releasing hormone (GnRH), Follicle-stimulating hormone (FSH), Luteinizing hormone (LH), Estrogen (E2) and progesterone (P4) were performed using (MyBioSource) commercial Elisa kits. The hormonal profile of all treatment groups of the buffalo heifers exhibited significant (P 0.05) variations as compared to the control group (G-1). These results indicate suppression in Nili-Ravi buffaloes' reproductive hormonal profile on exposure to P. multocida B:2 and its immunogens. This influence warrants that exposure to H.S may be a possible reason for delayed puberty and poor reproduction performance in Nili-Ravi buffaloes.


Resumo A pecuária é uma parte fundamental da indústria agrícola no Paquistão e contribui com 11,53% do PIB nacional. Entre as espécies de gado, os búfalos são considerados o ouro negro do Paquistão. Sendo os maiores produtores de leite em todo o mundo, os búfalos Nili-Ravi são os mais famosos. Os búfalos são afetados por muitas doenças endêmicas, entre as quais a septicemia hemorrágica (SH). Este estudo busca verificar os efeitos da exposição experimental de P. multocida B:2 (oral) e seus imunógenos, ou seja, LPS (oral e intravenoso) e OMP (oral e subcutâneo), nos perfis hormonais reprodutivos em búfalos Nili-Ravi. Amostras de soro repetidas foram coletadas da veia jugular de animais experimentais por 21 dias (0, 2, 4, 8, 12, 16, 20, 24, 36, 48, 72, 120, 168, 216, 264, 360, 456 e 504 horas). Os ensaios hormonais para determinar as concentrações séricas do hormônio liberador de gonadotrofina (GnRH), hormônio foliculoestimulante (FSH), hormônio luteinizante (LH), estrogênio (E2) e progesterona (P4) foram realizados usando kits comerciais Elisa (MyBioSource). O perfil hormonal de todos os grupos de tratamento das novilhas bubalinas apresentou variações significativas (P 0,05) em relação ao grupo controle (G-1). Esses resultados indicam supressão no perfil hormonal reprodutivo de búfalos Nili-Ravi na exposição a P. multocida B:2 e seus imunógenos. Essa influência garante que a exposição à SH possa ser uma possível razão para o atraso da puberdade e o baixo desempenho reprodutivo em búfalos Nili-Ravi.

ABSTRACT Background: Helicobacter pylori is an etiologic agent of gastroduodenal diseases. The microorganism, considered a type I carcinogen, affects about 50% of the global population. H. pylori virulence factors are determinant for the clinical outcome of the infection. The outer inflammatory protein A (oipA) gene encodes an outer membrane adhesin and is related to severe gastropathies, such as gastric cancer. Objective: The aim of this study was to evaluate the association of the oipA gene with the severity of gastroduodenal diseases in dyspeptic patients in region Central Brazil. Methods: The polymerase chain reaction (PCR) was used to determine the presence of H. pylori. Samples positives were used for molecular screening of the oipA gene. Gastropathies were categorized as non-severe and severe diseases. Results: Approximately 68% of patients had H. pylori and 36% were infected with H. pylori oipA+ strains. Infection was significantly associated in patients aged over 44 years (P=0.004). However, there was no association between oipA and patients' age (P=0.89). Approximately 46% of patients infected with oipA+ strains had some severe illness. Gastric adenocarcinoma was the most frequent severe gastropathy. The H. pylori oipA genotype was inversely associated with the severity of gastroduodenal diseases (OR=0.247, 95%CI: 0.0804-0.7149 and P=0.007). Conclusion: The characterization of possible molecular markers will contribute to personalized medicine, impacting the prognosis of patients.


RESUMO Contexto: Helicobacter pylori é um agente etiológico de doenças gastroduodenais. O microrganismo, considerado cancerígeno tipo I, afeta cerca de 50% da população mundial. Os fatores de virulência do H. pylori são determinantes para o desfecho clínico da infecção. O gene da proteína inflamatória externa A (oipA) codifica uma adesina da membrana externa e está relacionado a gastropatias severas, como o câncer gástrico. Objetivo: O objetivo deste estudo foi avaliar a associação do gene oipA com a gravidade das doenças gastroduodenais em pacientes dispépticos na região Brasil Central. Métodos: A reação em cadeia da polimerase (PCR) foi utilizada para determinar a presença de H. pylori. Amostras positivas foram utilizadas para triagem molecular do gene oipA. As gastropatias foram categorizadas como doenças não severas e severas. Resultados: Aproximadamente 68% dos pacientes apresentaram H. pylori e 36% estavam infectados com cepas H. pylori oipA+. A infecção foi significativamente associada em pacientes com idade superior a 44 anos (P=0,004). No entanto, não houve associação entre oipA e a idade dos pacientes (P=0,89). Aproximadamente 46% dos pacientes infectados com cepas oipA+ tiveram alguma doença severa. O adenocarcinoma gástrico foi a gastropatia severa mais frequente. O genótipo oipA de H. pylori foi inversamente associado à gravidade das doenças gastroduodenais (OR=0,247, IC95%: 0,0804-0,7149 P=0,007). ABSTRACT

La hemorragia producida por lesión de la arteria lingual en la base de la lengua por cirugías o por tumores es infrecuente. La mayor frecuencia en la indicación de abordajes transorales para tratar diferentes patologías que afectan la orofaringe requiere que el equipo quirúrgico tenga experiencia en el manejo de esta complicación. La ligadura de la arteria lingual en el cuello es una técnica quirúrgica muy eficaz para solucionar la hemorragia, pero es importante conocer las posibles variantes anatómicas que puede tener la arteria en su trayecto cervical. Debido a su baja incidencia se propone como objetivo describir dos casos clínicos de pacientes que tuvieron hemorragias graves por lesión de la arteria lingual en la base de la lengua, producidas por daño quirúrgico y por erosión por tumor. The bleeding caused by injury to the lingual artery at the base of the tongue due to surgery or tumors is infrequent. The increased frequency in the indication of transoral approaches to treat different pathologies affecting the oropharynx requires the surgical team to have experience in managing this complication. Ligation of the lingual artery in the neck is a very effective surgical technique to solve the bleeding; however, it is essential to be aware of the possible anatomical variants the artery may have in its cervical trajectory. Due to its low incidence, we propose to describe two clinical cases of patients who had severe bleeding due to a lesion of the lingual artery at the base of the tongue, produced by surgical damage and erosion due to a tumor. ABSTRACT

La reabsorción cervical externa es una patología poco frecuente y difícil de detectar, lo que complica el diagnóstico y pronóstico. El proceso destructivo provoca pérdida progresiva de tejido, comenzando en la superficie radicular y avanzando hacia la pulpa. Este tipo de reabsorción ocurre principalmente en la región cervical de la raíz dental pudiendo confundirse con lesión de caries radicular. El objetivo del reporte de caso es ilustrar al equipo de salud la existencia de esta condición y destacar la importancia del diagnóstico temprano y diferencial para evitar llegar a la pérdida del diente afectado.


External cervical resorption is a rare and difficult-to-detect condition, which complicates diagnosis and prognosis. The destructive process causes progressive loss of tissue, starting at the root surface and advancing towards the pulp. This type of resorption primarily occurs in the cervical region of the dental root and can be confused with root caries. The case report aims to inform the healthcare team about the presence of this condition and emphasizes the significance of early and differential diagnosis to prevent the loss of the affected tooth.

Introdução: O traumatismo dentário é uma ocorrência frequente durante a infância e adolescência. Geralmente, o diagnóstico acontece de forma tardia, devido à falta de conhecimento da população. Cada tipo de trauma necessita de uma conduta única e especializada. Nos casos de comprometimento pulpar, é necessário cuidado quanto à escolha da técnica a ser empregada, bem como a seleção da medicação intracanal que, por sua vez, apresenta grande interferência no sucesso do tratamento. Não obstante o avanço das técnicas de instrumentação, a troca da medicação entre as sessões nesses casos é mandatória. Relato de caso: Um paciente, sexo masculino, em idade escolar (15 anos) apresentou-se ao serviço de saúde bucal com relato de escurecimento e dores na mastigação após trauma dentário durante a infância. Os testes de vitalidade pulpar revelaram necrose pulpar, com resultado negativo no teste térmico e positivo na percussão. O exame radiológico evidenciou uma lesão apical, juntamente com reabsorção externa no elemento 21, confirmado em tomografia. O paciente foi submetido a um tratamento endodôntico. Resultado: Após o tratamento, foi solicitado novo exame tomográfico, em que se evidenciou um processo de cicatrização óssea no periápice e reabsorção por substituição na lesão cervical. Conclusão: A partir deste caso clínico, pôde-se evidenciar a importância do diagnóstico adequado, do manejo e da técnica endodôntica eficaz na resolução de problemas decorrentes de traumas dentários em pacientes jovens.


Introduction: Dental trauma is a frequent occurrence during childhood and adolescence. Generally, diagnosis happens late, due to lack of knowledge among the population. Each type of trauma requires a unique and specialized approach. In cases of pulp involvement, care must be taken when choosing the technique to be used, as well as the selection of intracanal medication, which, in turn, has a major impact on the success of the treatment. Despite advances in instrumentation techniques, changing medication between sessions in these cases is mandatory. Case report: A male patient of school age (15 years old) presented to the oral health service with reports of darkening and pain when chewing following dental trauma during childhood. Pulp vitality tests revealed pulp necrosis, with a negative result in the thermal test and positive in percussion. The radiological examination showed an apical lesion, together with external resorption in element 21, confirmed on tomography. The patient underwent endodontic treatment. Result: After treatment, a new tomographic examination was requested, which showed a process of bone healing in the periapex and resorption by replacement in the cervical lesion. Conclusion: From this clinical case, it was possible to highlight the importance of adequate diagnosis, management, and effective endodontic technique in solving problems arising from dental trauma in young patients.

ABSTRACT

Pseudomonas aeruginosa en una bacteria Gram negativa no fermentadora que produce diversos tipos de infecciones severas en inmunocompetentes e inmunodeprimidos. Una de estas infecciones es la otitis externa maligna, la cual se presenta principalmente en personas con diabetes mellitus y puede tener una evolución tórpida cursando con osteomielitis de base de cráneo y parálisis de nervios craneales. El tratamiento es individualizado y principalmente conservador con antibioticoterapia guiada por cultivo. La bacteria aislada en la mayoría de los casos reportados es sensible a los antibióticos anti-pseudomónicos. Reportamos un caso de presentación inusual de otitis externa maligna por Pseudomonas aeruginosa resistente a carbapenémicos con evidencia sugerente de compromiso bilateral y en el cual se aisló al mismo germen en urocultivo y hemocultivos, lo que indicaría una diseminación hematógena del microorganismo.


Pseudomonas aeruginosa is a non-fermenting Gram-negative bacterium that produces several types of severe infections in immunocompetent and immunosuppressed patients. One of these infections is malignant otitis externa, which occurs mainly in people with diabetes mellitus and can have a torpid evolution coursing with osteomyelitis of skull base and cranial nerve palsies. Treatment is individualized and mainly conservative with culture-guided antibiotic therapy, with isolated pseudomonas being sensitive to anti-pseudomonal antibiotics in the majority of reported cases. We report a case of unusual presentation of malignant otitis externa caused by Pseudomonas aeruginosa resistant to carbapenems with suggestive evidence of bilateral involvement and in which the same germ was isolated in urine and blood cultures, which would indicate hematogenous dissemination of the microorganism.

ABSTRACT

Background: Otitis externa is frequently seen illness by ENT practitioners predominantly in children and old age people. The severity sometimes varies from it ranges in severity from a trivial infection to dangerous malignant otitis externa. Furthermore, due to absurd consumption of antibiotics, though there is a fall in development of complications, there is a rise in resistance to various infectious organisms. Mostly, we start treating patients with empirical therapy where sometimes, it leads to occurrence of various antibiotic-resistant infections. For that reason, recent evidence about the organism which is mostly involved in infection and also most importantly the pattern of their response to various pharmacological agents is crucial for the sensible use of the medications for the management. Aim and Objectives: Our study was planned to evaluate and find out the causative organism which is most commonly seen in otitis externa, particularly focusing on drug sensitivity for those organisms, so that efficient management of otitis externa can be done. Materials and Methods: It is a retrospective study conducted in acute otitis externa patients from Novemver 2019 to January 2020. Patients who all presented with ear discharge within 3 months of period, samples were collected with clinically diagnosed acute otitis externa and sent for culture in blood agar, chocolate agar, and Mcconkey medium in vitro and drug sensitivity pattern was identified for individual organisms. Inclusion criteria: Patients more than 18 years having ear discharge within 3 months were included in the study. Exclusion criteria: Chronically discharging ear >3 months were excluded from the study. Results: Pseudomonas organism was frequently encountered pathogen in our study, trailed by Staphylococcus aureus. The organisms identified were very much sensitive to drugs like fluoroquinolones particularly Ciprofloxacin, and Aminoglycosides like Amikacin and almost all were responding to treatment with Piperacillin-Tazobactam, these drugs have an cidal activity over pathogens and are suitable for any age and safety-wise better too. Conclusion: Intermittent assessment and awareness of the bacteriological summary and their drug response profile in a particular geographical area will only help in rational drug prescription. Furthermore, rational usage of drugs will help in reducing any morbidity due to infection which may sometimes develop when treated inappropriately, further decreasing development of dangerous drug resistance.

Introduction: As a health science, dentistry seeks to raise self-esteem and improve patients' quality of life. However, the emergence of the aesthetic procedure "ear shut," which proposes correcting protruding ears without surgery, has raised ethical and legal doubts in the dental profession, especially concerning the limits of professional activity. Objective: To conduct a survey of the laws, norms, and resolutions on the area of performance of dental surgeons, as well as to discuss the limits and consequences of their extrapolation from the perspective of the procedure propagated as "ear shut." Method: A search was carried out for administrative norms on the Federal Council of Dentistry (CFO) websites and the Federal Council of Medicine and legal provisions on the Portal da Legislação website. Results: In the civil sphere, the disclosure of procedures such as the "ear shut" can characterize a promise of result and lead to judicial accountability. Exceeding professional boundaries constitutes an illegal exercise and a crime under the Brazilian Penal Code. In addition to not covering the area of clinical practice of dentists, at the administrative level, CFO Resolutions No. 198/2019 and No. 230/2020 emphasize that ear procedures are not part of the scope of procedures relevant to Dentistry and may lead to ethical infractions and consequent administrative proceedings. Conclusion: At present, based on the skills, prohibitions, rights, and duties of dentists, it can be stated that performing the "ear shut" by these professionals confronts civil, criminal, and administrative obligations.


Introdução: Como ciência da saúde, a Odontologia busca elevar a autoestima e melhorar a qualidade de vida dos pacientes. Entretanto, o surgimento do procedimento estético "ear shut", que propõe a correção da orelha em abano sem cirurgia, gerou dúvidas de ordem ética e legal na classe odontológica, sobretudo no que diz respeito aos limites de atuação profissional. Objetivo: Realizar o levantamento das leis, normativas e resoluções sobre a área de atuação dos cirurgiões-dentistas, bem como discutir os limites e as consequências de sua extrapolação sob a perspectiva do procedimento propagado como "ear shut". Método: Foi realizada uma busca de normas administrativas nos sites do Conselho Federal de Odontologia (CFO) e do Conselho Federal de Medicina, bem como de dispositivos legais no site Portal da Legislação. Resultados: Em âmbito cível, a divulgação de procedimentos como o "ear shut" pode caracterizar promessa de resultado e levar à responsabilização judicial. Ultrapassar os limites profissionais constitui exercício ilícito e constitui crime segundo o Código Penal Brasileiro. Além de não abranger a área de atuação clínica dos cirurgiões-dentistas, no âmbito administrativo, as Resoluções do CFO N° 198/2019 e N° 230/2020 enfatizam que procedimentos na orelha não compõem o escopo de procedimentos pertinentes à Odontologia, podendo levar a infrações éticas e consequente processo administrativo. Conclusão: No momento atual, com base nas competências, vedações, direitos e deveres dos cirurgiões-dentistas, pode-se afirmar que a realização do "ear shut" por estes profissionais confronta obrigações de ordem cível, penal e administrativa.

ABSTRACT

Skull base osteomyelitis is a rare, refractory, and potentially fatal disease primarily caused by otogenic and sino rhinogenic infections. At times, it can mimic neoplasia complicating the diagnosis. With the use of antibiotics, advancements in diagnostic methods, and skull base surgical techniques, the mortality rate has significantly improved. However, the successful diagnosis and treatment of the disease is still challenging due to delayed diagnosis, lengthy treatment course, a tendency for relapse and lack of guidelines. Therefore, this article aims to review the progress in the diagnosis and treatment of skull base osteomyelitis.

In this paper, the latest advances in etiology, pathogenesis, clinical manifestations, diagnosis and treatment were reviewed based on previous literature, providing reference for clinical diagnosis, treatment and future research.

ABSTRACT

La coloración rosa de los dientes puede originarse por diferentes factores. En el ámbito forense se ha descrito al fenómeno denominado post mortem pink teeth como un signo asociado a muertes violentas de etiología diversa. En la práctica clínica también es posible observar pacientes con dientes rosados, fre-cuentemente ocasionados por traumatismos o iatro-genia proveniente de ortodoncia, cuyo mecanismo de producción obedece a distintas etiopatogenias, destacándose las reabsorciones dentinarias inter-nas, cemento-dentinarias externas y calcificaciones dentinarias. El presente artículo expone el caso de un individuo adulto con antecedente de trauma óseo-dentario por accidente vial que, luego de un prolon-gado tiempo, asiste al Servicio de Urgencias Odon-tológicas y Orientación de Pacientes de la Facultad de Odontología de la Universidad de Buenos Aires, en donde se le detecta, a modo de hallazgo exploratorio, una ostensible coloración rosada en el canino infe-rior derecho. La situación motivó un pormenorizado abordaje clínico y radiográfico, indagando respecto a los probables factores que intervinieron en su ge-neración y desarrollo (AU)


The pink coloration of the teeth can be caused by dif-ferent factors. In the forensic field, the phenomenon called post mortem pink teeth has been described as a sign associated with violent deaths of various etiology. In clinical practice, it is also possible to ob-serve patients with pink teeth, frequently caused by trauma or iatrogenesis from orthodontics, whose production mechanism is due to different etiopatho-genesis, highlighting internal dentin resorption, ex-ternal cemento-dentinal resorption and dentin calci-fications. This article presents the case of an adult individual with a history of bone-dental trauma due to a road accident who, after a long time, attends the Dental Emergency and Patient Guidance Service of the Faculty of Dentistry of the University of Bue-nos Aires, where an ostensible pink coloration was detected in the lower right canine as an exploratory finding. The situation motivated a detailed clinical and radiographic approach, inquiring about the probable factors that intervened in its generation and development (AU)

Abstract Objective: To determine the diagnostic accuracy of Necrotizing Otitis Externa (NOE) based on radiologic studies. Methods: The PubMed, Cochrane, Embase, Web of Science, SCOPUS, and Google Scholar databases were searched. True-positive and false-negative results were extracted for each study. Methodological quality was evaluated using the Quality Assessment of Diagnostic Accuracy Studies 2 (QUADAS-2) tool. Results: The included studies contained data on 37 studies diagnosed with NOE. The sensitivity of gallium-67, technetium-99m, and Magnetic Resonance Imaging (MRI) was 0.9378 (0.7688-0.9856), 0.9699 (0.8839-0.9927), and 0.9417 (0.6968-0.9913), respectively. For Computed Tomography (CT), the positive criteria consisted of bony erosion alone and bony erosion plus any soft tissue abnormality. The sensitivity of CT based only on bony erosion was 0.7062 (0.5954-0.7971); it was higher 0.9572 (0.9000-0.9823) when based on bony erosion plus any soft tissue abnormality. Conclusion: The diagnostic sensitivity of technetium-99m, gallium-67, and MRI was favorable. On CT, the presence of bony erosion may be a useful diagnostic marker of NOE, but the diagnostic sensitivity will be even higher if the criterion of any soft tissue abnormality is also included; however, care should be taken when interpreting the results. Our study demonstrates the potential utility of radiology studies for diagnosing NOE, but their lack of specificity must be considered, and standardized anatomic criteria are still needed. La gestión de la información de salud del paciente, así como de los diferentes servicios que se brindan en los centros de atención de salud, constituyen elementos cruciales para prestar un servicio de salud de buena calidad. El Sistema de Información Hospitalaria XAVIA HIS, constituye una solución integral para la gestión médica de hospitales y centros de salud. En el módulo de Consulta externa se gestiona la información referente a la atención a pacientes ambulatorios en diferentes especialidades. El objetivo del presente trabajo es describir las principales funcionalidades y especialidades médicas incluidas en el módulo Consulta externa del sistema XAVIA HIS. El desarrollo estuvo guiado por la metodología de desarrollo Proceso Ágil Unificado. variante UCI y fueron empleadas las tecnologías, herramientas y lenguajes que forman parte de la arquitectura del sistema definida por el Centro de Informática Médica, entre las cuales se pueden mencionar: Java Enterprise Edition 6 como plataforma de programación para el desarrollo y la ejecución del sistema, como sistema gestor de base de datos se empleó PostgreSQL 10, como herramienta de modelado Visual Paradigm para UML, la notación BPMN 2.0 (Business Process Management Notation) y el Lenguaje Unificado de Modelado (UML) y el estándar HL7 CDA® (Clinical Document Architecture) para homogeneizar la arquitectura de los documentos clínicos. El desarrollo de este módulo refuerza la base de conocimientos necesaria para la toma de decisiones clínicas y administrativas, mejora el acceso a la información y la calidad de la asistencia a los pacientes.


The patient's health information management, as well as different services provided in health care centers, constitutes crucial elements to provide a good quality health service. The Hospital Information System XAVIA HIS establishes a comprehensive solution for hospitals and health centers medical management. The Outpatient module manages the information regarding outpatient care in different specialties. This paper aims to describe the main functionalities and medical specialties included in the Outpatient module of the XAVIA HIS system. The development was guided by the AUP development methodology (an UCI variant), and to achieve it, the technologies, tools and languages used are part of the system architecture defined by the CESIM and mentioned as follow: Java Enterprise Edition 6 platform as the Runtime Environment, PostgreSQL 10 as the database management system, Visual Paradigm as modeling tool for UML, the BPMN 2.0 notation (Business Process Management Notation), the Unified Modeling Language (UML) and the HL7 CDA® (Clinical Document Architecture) standard to standardize the architecture of clinical documents. This module development reinforces the knowledge base necessary for clinical and administrative decision-making, improves access to information and patients' care quality.

La nariz es estructuralmente compleja, y esta complejidad da como resultado variaciones de forma nasal. El estudio tuvo como objetivo determinar las desviaciones septales nasales que ocurren en relación con las deformidades nasales externas. Se realizó un estudio transversal en el departamento de oído, nariz y garganta, en nuestro hospital. Todos los individuos que tienen desviación septal con deformidad nasal externa de noviembre de 2017 a noviembre de 2018. Esos pacientes serán evaluados mediante un examen clínico integral de la oreja, la nariz y la garganta. Los síntomas del paciente se clasifican con el cuestionario de prueba china nasal -22. Las deformidades septales se clasificaron utilizando la clasificación Mladina modificada. Las deformidades nasales externas se clasificaron empleando la clasificación de Yong Jo Jang. Aproximadamente, el 43% eran hombres y el 57% eran mujeres. Alrededor del 90% de los pacientes de 21 años a 50 años. Alrededor del 58% de los pacientes eran sintomáticos, mientras que el resto del 42% no tenía síntomas. Al correlacionar los síntomas con el tipo de desviación, se encontró significativamente asociación (P = 0.05). La mayoría de las personas se encuentran en el grupo de mediana edad. Tipo II y IV son los tipos más comunes de NSD, mientras que el tipo I es un final común. El encuentro notable de nuestro estudio es que los pacientes no tenían deformidad


The nose is structurally complex, and this complexity results in nasal shape variations. The study aimed to determine the nasal septal deviations occurrence in relation to external nasal deformities. A crosssectional study was conducted in Department of Ear, Nose and Throat, in our hospital. All the individuals having septal deviation with external nasal deformity from November 2017 to November 2018. Those patients be evaluated by comprehensive clinical examination of ear, nose and throat. Patient's symptoms are rated with Sino-Nasal Test -22 questionnaire. Septal deformities were classified using Mladina classification modified. External nasal deformities were classified employing Yong Jo Jang's classification. Approximately, 43% were males and 57% were females. About 90% of patients aged from 21 years to 50 years. About 58% of patients were symptomatic while the rest 42% were without symptoms. On correlating the symptoms with the type of deviation it was found significantly association (P=0.05). Majority of individuals are in the middle age group. Type II and IV are the most common types of NSD whereas type I is a common END. Noteworthy finding of our study is patients had no deformity.

Resumen Introducción: la identificación de los pacientes con mayor riesgo de progresar a falla renal es fundamental para la planeación del tratamiento en la enfermedad renal crónica, pero no ha podido llevarse a cabo consistentemente. Los modelos de predicción podrían ser una herramienta útil, sin embargo, su usabilidad en la Enfermedad Renal Crónica es limitada hasta ahora y no se comprenden muy bien las barreras y limitaciones. Métodos: se desarrolló una revisión de alcance de la literatura disponible sobre modelos predictivos de falla renal o reglas de pronóstico en pacientes con Enfermedad Renal Crónica. Las búsquedas se realizaron sistemáticamente en Cochrane, Pubmed y Embase. Se realizó una revisión ciega e independiente por dos evaluadores para identificar estudios que informaran sobre el desarrollo, la validación o la evaluación del impacto de un modelo construido para predecir la progresión al estadio avanzado de la enfermedad renal crónica. Se realizó una evaluación crítica de la calidad de la evidencia proporcionada con el sistema GRADE (Grading of Recommendations Assessment, Development and Evaluation). Resultados: de 1279 artículos encontrados, fueron incluidos 19 estudios para la síntesis cualitativa final. La mayoría de los estudios eran primarios, con diseños observacionales retrospectivos y unos pocos correspondieron a revisiones sistemáticas. No se encontraron guías de práctica clínica. La síntesis cualitativa evidenció gran heterogeneidad en el desarrollo de los modelos, así como en el reporte de las medidas de desempeño global, la validez interna y la falta de validez externa en la mayoría de los estudios. La calificación de la evidencia arrojó una calidad global baja, con inconsistencia entre los estudios e importantes limitaciones metodológicas. Conclusiones: la mayoría de los modelos predictivos disponibles, no han sido adecuadamente validados y, por tanto, se consideran de uso limitado para evaluar el pronóstico individual del paciente con enfermedad renal crónica. Por lo tanto, se requieren esfuerzos adicionales para centrar el desarrollo e implementación de modelos predictivos en la validez externa y la usabilidad y disminuir la brecha entre la generación, la síntesis de evidencia y la toma de decisiones en el ámbito del cuidado del paciente.


Abstract Background: the identification of patients at higher risk of progressing to kidney failure is essential for treatment planning in chronic kidney disease, but it has not been possible to do this consistently. Predictive models could be a useful tool, however, their usability in chronic kidney disease is limited and the barriers and limitations are not well understood. Methods: a scoping review of the available literature on ESRD predictive models or prognostic rules in chronic kidney disease patients was developed. Searches were systematically executed on Cochrane, MEDLINE, and Embase. a blind and independent review was carried out by two evaluators to identify studies that reported on the development, validation, or impact assessment of a model constructed to predict the progression to an advanced stage of chronic kidney disease. A critical evaluation of the quality of the evidence provided with the GRADE system (Grading of Recommendations Assessment, Development and Evaluation) was made. Findings: of 1279 articles found, 19 studies were included for the final qualitative synthesis. Most of the studies were primary, with retrospective observational designs and a few corresponded to systematic reviews. No clinical practice guidelines were found. The qualitative synthesis showed high heterogeneity in the development of the models, as well as in the reporting of global performance measures, internal validity, and the lack of external validity in most of the studies. The evidence rating was of low overall quality, with inconsistency between studies and important methodological limitations. Conclusions: most of the available predictive models have not been adequately validated and, therefore, are of limited use to assess the individual prognosis of patients with chronic kidney disease. Therefore, additional efforts are required to focus the development and implementation of predictive models on external validity and usability and bridge the gap between generation, synthesis of evidence, and decision-making in the field of patient care.

ABSTRACT

Defeitos de orelha são frequentes e de etiologias diversas. Ainda assim, a reconstrução de orelha permanece um desafio dentro da cirurgia plástica reconstrutiva, principalmente devido a anatomia e a escassez de tecido local. Embora pouco utilizado, o retalho tubular retroauricular se apresenta como alternativa para reconstrução da hélice e lóbulo. No presente estudo os autores descrevem a técnica operatória, bem como relatam um caso em que foi utilizado. Realizou-se uma avaliação do resultado operatório de quatro casos de reconstrução de orelha utilizando o retalho tubular retroauricular, através de questionários encaminhados para avaliadores leigos e cirurgiões plásticos. O resultado estético final foi classificado como bom ou excelente por 35% dos avaliadores leigos e 50% dos cirurgiões plásticos. Já o resultado operatório foi avaliado como bom ou excelente por 70% dos leigos e 80% dos cirurgiões plásticos. Os resultados permitem concluir que a técnica do retalho tubular retroauricular para reconstrução de defeitos da borda de hélice pode ser indicada, com resultados satisfatórios.


Ear defects are frequent and result of many etiologies, even though ear reconstruction remains a challenge in plastic reconstructive surgery due to anatomy and local tissue paucity. Despite being rarely used, the tubular retroauricular flap presents as an alternative for helix and lobule reconstruction. In this article, the authors describe the operative technique and report a case in which it was used. Also, plastic surgeons and laypeople rated the operative results of four cases of ear reconstructions using the tubular retroauricular flap. The final esthetic result was rated as good or excellent by 35% of laypeople and 50% of plastic surgeons, whereas the operative result was rated as good or excellent by 70% of laypeople and 80% of plastic surgeons. Thereby the findings and authors' experience, we can recommend the tubular retroauricular flap technique for reconstructing ear helix border defects.

Introducción: Los antinflamatorios no esteroideos son los medicamentos más recetados por reumatólogos y traumatólogos. Pero a pesar de tener una eficacia similar para controlar la inflamación y el dolor, los diferentes antinflamatorios no esteroideos disponibles presentan variabilidad en su perfil de seguridad. Objetivo: Brindar una panorámica sobre la prescripción de protectores gástricos en pacientes reumatológicos, en dos centros hospitalarios, desde una perspectiva gastroenterológica. Métodos: La investigación se inscribe en el paradigma cuantitativo con un estudio observacional. Se conformaron dos grupos de pacientes, uno de ellos provenientes del hospital A y el segundo grupo pertenecía al hospital B. Discusión: Los gastroprotectores se han convertido en los fármacos de mayor demanda en las farmacias comunitarias de Ecuador, y los más prescritos son los inhibidores de la bomba de protones y en menor frecuencia los antihistamínicos H2. Respecto a los gastroprotectores más frecuentemente prescritos, según nuestro estudio, estos valores podrían obedecer a la disponibilidad y los costos de la famotidina y el omeprazol con respecto a otros medicamentos de probada eficacia (ansoprazol, pantoprazol, rabeprazol y ranitidina). Conclusiones: Durante los últimos años, el uso de los fármacos gastroprotectores en América Latina ha experimentado un importante desarrollo, con la observancia de normas y guías clínicas de manejo de casos que ofrecen recomendaciones importantes al respecto. Por tanto, desde una perspectiva gastroenterológica, para tener éxito, es indispensable procurar un conocimiento de estos aportes y evidencias científicas(AU)


Introduction: Nonsteroidal anti-inflammatory drugs are the most prescribed medications by rheumatologists and traumatologists. However, despite having similar efficacy in controlling inflammation and pain, the different available nonsteroidal anti-inflammatory drugs show variability in their safety profile. Objective: To provide an overview of the prescription of gastric protectors in rheumatological patients, in two hospital centers, from a gastroenterological perspective. Methods: The research is part of the quantitative paradigm with an observational study. Two groups of patients were formed, one of them from hospital A and the second group belonged to hospital B. Discussion: Gastroprotectors have become the drugs in greatest demand in community pharmacies in Ecuador, the most prescribed being Proton Pump Inhibitors (IBPS) and, to a lesser extent, H2 antihistamines (anti-H2). Regarding the most frequently prescribed gastroprotectors, according to our study, these values ​​could be due to the availability and costs of famotidine and omeprazole compared to other drugs with proven effectiveness, such as ansoprazol, pantoprazole, rabeprazole and ranitidine. Conclusions: In recent years, the use of gastroprotective drugs in Latin America has undergone significant development, with the observance of clinical case management norms and guidelines that offer important recommendations in this regard. Therefore, from a gastroenterological perspective, to be successful, it is essential to seek knowledge of these contributions and scientific evidence(AU)

RESUMEN Las enfermedades neurológicas se han convertido en una preocupación creciente, considerándose causa importante de morbimortalidad tanto en países desarrollados como en aquellos en desarrollo. Se realizó esta investigación con el objetivo de caracterizar la actividad asistencial del servicio de Neurología del Hospital Clínico-Quirúrgico Celia Sánchez Manduley. Se realizó un estudio observacional, descriptivo en el servicio de Neurología del Hospital Clínico-Quirúrgico Celia Sánchez Manduley, en el periodo de enero a diciembre de 2019. El universo estuvo constituido por 3968 pacientes, atendidos en consulta externa y hospitalización. Los resultados obtenidos se distribuyeron en frecuencias absolutas y por cientos. Se tomaron en cuenta las siguientes variables: edad, sexo, municipio, grupo diagnóstico y origen de la interconsulta. Predominó el sexo femenino (57,7 %) y el grupo de edad de 60 años y más (37,2 %). La mayoría de los pacientes fueron manzanilleros (36,4 %). La cefalea fue el grupo diagnóstico más representativo (23,9 %), y la epilepsia fue el principal diagnóstico con que ingresaron los pacientes (32,8 %). El servicio que aportó mayores solicitudes de interconsultas para un 66,8 % fue Medicina Interna. Los motivos de consulta y las características de los pacientes hospitalizados no difieren en gran medida de los presentados por otros autores. Los motivos de consulta más frecuentes fueron la cefalea y la epilepsia. El grupo de edad con mayor demanda de asistencia neurológica fueron pacientes mayores de 60 años, con predominio del sexo femenino. El mayor número de hospitalizaciones correspondió a la epilepsia y la enfermedad de Parkinson.


ABSTRACT Neurological diseases have become a growing concern, being considered a major cause of morbidity and mortality in both developed and developing countries. This research was carried out with the aim of characterizing the care activity of the Neurology service of the Celia Sánchez Manduley Clinical-Surgical Hospital. An observational, descriptive study was conducted in the Neurology service of the Celia Sánchez Manduley Clinical-Surgical Hospital, in the period from January to December 2019. The universe consisted of 3968 patients, treated in outpatient consultation and hospitalization. The results obtained were distributed in absolute frequencies and by hundreds. The following variables were taken into account: age, sex, municipality, diagnostic group and origin of the interconsultation. Female sex (57.7%) and the age group of 60 years and over (37.2%) predominated. The majority of patients were manzanilleros (36.4 %). Headache was the most representative diagnostic group (23.9%), and epilepsy was the main diagnosis with which patients were admitted (32.8%). The service that provided the highest requests for interconsultations for 66.8% was Internal Medicine. The reasons for consultation and the characteristics of hospitalized patients do not differ greatly from those presented by other authors. The most frequent reasons for consultation were headache and epilepsy. The age group with the highest demand for neurological assistance were patients over 60 years of age, with a predominance of the female sex. The highest number of hospitalizations corresponded to epilepsy and Parkinson's disease.


RESUMO As doenças neurológicas tornaram-se uma preocupação crescente, sendo consideradas uma das principais causas de morbidade e mortalidade nos países desenvolvidos e em desenvolvimento. O universo era composto por 3.968 pacientes, tratados em consulta ambulatorial e internação. Os resultados obtidos foram distribuídos em frequências absolutas e por centenas. Foram levadas em consideração as seguintes variáveis: idade, sexo, município, grupo diagnóstico e origem da interconsulção. Predominaram o sexo feminino (57,7%) e a faixa etária de 60 anos ou mais (37,2%) predominaram. A maioria dos pacientes foi manzanilleros (36,4 %). A dor de cabeça foi o grupo diagnóstico mais representativo (23,9%), e a epilepsia foi o principal diagnóstico com o qual os pacientes foram internados (32,8%). O serviço que atendeu mais pedidos de interconsultações para 66,8% foi a Medicina Interna. As razões para a consulta e as características dos pacientes internados não diferem muito das apresentadas por outros autores. Os motivos mais frequentes para a consulta foramdor de cabeça e epilepsia. A faixa etária com maior demanda por assistência neurológica foram pacientes com mais de 60 anos de idade, com predominância do sexo feminino. O maior número de internações correspondeu à epilepsia e doença de Parkinson.

ABSTRACT

Objetivo: Describir el resultado de la técnica de fijación muscular orbicular externa en la Blefaroplastia inferior. Método: Se realizó una investigación descriptiva, prospectiva en pacientes que asistieron a la consulta de Cirugía Plástica del Hospital "Camilo Cienfuegos" entre enero 2018 a enero 2019, la muestra estuvo constituida por 43 pacientes. Para diagnosticarlos se utilizó la clasificación de Ginsbach, con criterios de exclusión el hábito de fumar. Se tuvo en cuenta variables como edad, sexo, deformidad palpebral inferior, complicaciones en el post operatorio a las 24 h y a los 30 días, y grado de satisfacción de los pacientes intervenidos. Se utilizaron métodos de revisión documental, observación, análisis y síntesis y empírico (encuesta), utilizadas para evaluar los resultados. La investigación se realizó siguiendo procedimientos éticos. Resultados: Mostraron que la mayor cantidad de pacientes estuvo en el grupo edad entre 46-50 años todos femeninos, el exceso de piel y las patas de gallina fueron las deformidades que predominaron. La complicación que se presentó fue el sangramiento posoperatorio resuelto en las primeras horas, 42 pacientes refirieron estar satisfechos. Conclusiones: La blefaroplastia es una técnica que se ha venido modificando junto a la necesidad de brindar procedimientos que ofrezcan seguridad y efectividad, en cuanto a la corrección de deformidades en el parpado inferior, con un adecuado tiempo de recuperación, resultados con menor riesgo de complicaciones(AU)


Objective: To describe the outcome of the external orbicularis oris muscle fixation technique in lower blepharoplasty. Methods: A descriptive, prospective research was conducted in patients who attended the Plastic Surgery consultation of the Hospital "Camilo Cienfuegos" from January 2018 to January 2019, the sample consisted of 43 patients. In order to diagnose them, the Ginsbach classification was used, with smoking as exclusion criteria. Variables such as age, gender, lower palpebral deformity, postoperative complications at 24 h and 30 days, and degree of satisfaction of the operated patients were taken into account. Documentary review, observation, analysis and synthesis and empirical (survey) methods were used to evaluate the results. The research was carried out following ethical procedures. Results: They showed that the greatest number of patients was in the age group between 46-50 years old, all of them female; excess skin and goose bumps were the predominant deformities. The complication that occurred was postoperative bleeding resolved in the first hours, 42 patients reported to be satisfied. Conclusions: Blepharoplasty is a technique that has been modified along with the need to provide procedures that offer safety and effectiveness, in terms of correction of deformities in the lower eyelid, with adequate recovery time, results with lower risk of complications(AU)
